the idea of the connection of anxiety with the act of birth belongs to Sigmund Freud. In 1900, in the notes to the "Interpretation of dreams” he noted that the anxiety associated with the act of birth, before reflecting on the fantasies of intrauterine existence.
Rank wrote a book “birth Injury", dedicating it to Freud. In this book fear Chapter is devoted to the “Infantile fear”: “… in fact, the affect of fear from the act of birth in a latent form remains and continues to operate any coincidence that as or more often "symbolically" - it "reminds" is used to re-respond again to this hidden passion,” [26, p. 47].
In this case, Rank is talking about a random night fear. Fear, according to Rank, appears as response to the initial fear, experienced at birth. Reasoning in the logic of “from sensations to affect”, Otto Rank repeats and the theory of trauma, and the theory of the origin of anxiety in the result transformed physiological arousal. Otto Rank believed that

Freud says that we tend to believe that in a state of anxiety is the reproduction of the trauma of birth.
the Trauma of birth - not just a metaphor. In a single concept contains two dictionary values.
Freud's trauma of birth -
“the First event that causes the person anxiety is birth; objectively, it means separation from the mother and can be likened castration of the mother (in accordance with the equation child = penis)" [40, p. 271].
